The 2024/2025 District Grant report form listed below is a .pdf fillable form. While you may view this form online, you will need to download it in order to enter and save the entered information and submit it for reimbursement.

*To download the .pdf form*: Click on the "District Grant Reporting Form" box which should make the form visible. but not usable - download the form.

Save the downloaded .pdf form renaming it to make it distinctive to your club, ie. norwinreport.pdf.

Now you're ready to proceed with filling out the application. While most .pdf reader applications will work with this form nicely, some are not designed exactly to Adobe standards. If you're having problems, get the free Adobe Reader at https://get.adobe.com/reader/ and see if that works better for you. Once you've entered your responses, save it again (it doesn't hurt to save it a few times while working on it). The entries do not save themselves, you need to save the document with the entries.

Once you've completed it you may want to print out a copy for yourself. Email the uniquely named report form to us at [email protected] along with scanned or digital copies of all applicable receipts.

Look for an emailed confirmation that we have received your report which should come within 48 hours of your submission. If you don't see one, it is very possible we have not received it. If you don't hear from us, contact us ASAP.
- Once you receive the reimbursement check, please deposit it to your clubs bank account right away to ensure proper close out of your grant
